A search warrant reveals that a Wisconsin employee of USIC, a company based in Indiana with locations across the U.S., failed to accurately mark the location of a gas line. Instead, they marked a spot on the sidewalk about 25 feet away, where no gas line actually existed.
The warrant was issued to investigate the public street for evidence related to a second-degree reckless homicide charge in connection with the death of Sun Prairie Volunteer Fire Department Captain Cory Barr. He died on July 10 when gas leaking from a ruptured 4-inch natural gas line ignited, causing a massive explosion. Another firefighter, Ryan Welch, was seriously injured in the incident.
The search sought electronic devices and data from equipment used during the underground boring operation at the time the gas line was struck. It also included items related to fiber communication conduits and sections of the gas line itself.
According to the search warrant affidavit:
Buildings at 100 and 104 W. Main St. in Sun Prairie exploded approximately 40 minutes after a gas leak was reported to 911. The blast destroyed properties from 100 to 113 W. Main St., on both sides of the street.
Valentine Cociuba of VC Tech, which was hired by Verizon contractor Bear Communications to install a fiber communication line, told investigators that Bear had confirmed the Diggers Hotline had been called on July 9. This allowed VC Tech to proceed with horizontal-directional boring along Bristol Street and Main Street.
Cociuba explained that work began after reviewing the markings made by USIC and developing a plan. However, he and his team detected a gas smell near the southwest corner of the Bristol-Main Street intersection and immediately called 911. He took a photo of the area before evacuating, which showed no “T†markings indicating an intersecting utility like a gas line on the pavement. Later investigations confirmed that no such markings were present above the actual gas line. Instead, there was a yellow painted “T†on the sidewalk about 25 feet north of the damaged pipe — but no gas line was located there.
A separate company was brought in to verify the gas line locations and confirmed these findings.
State Department of Criminal Investigation Agent Ryan Windorff was given a map by We Energies showing the location of gas utilities in the area. He noted that the gas line was in the road, not on the sidewalk as marked by USIC.
